• We determined visual and automated REM sleep without atonia (RSWA) diagnostic cut-offs for idiopathic REM sleep behavior disorder (iRBD) patients with comorbid obstructive sleep apnea (OSA) during split-night polysomnography.• Combining analysis of phasic muscle activity burst durations with standard visual and automated methods improved sensitivity and specificity for iRBD diagnosis.• iRBD RSWA cutoffs were similar to those previously determined for PD-RBD, possibly indicating a similar underlying neurodegenerative process.
